If you only plan to schedule if insurance says they cover our services and you have not checked your benefits, please do not schedule here. Instead, complete our inquiry form to receive directions for checking your insurance benefits first.


Requesting an Appointment

To schedule, we require a credit card on file. The credit card is used for costs insurance doesn’t cover and for late cancellations / no shows. Late cancellations (within 24 hours of the session time) are charged late fees outlined in our office policies and located on our website, which you’ll receive with intake paperwork upon acceptance of your requested appointment.

We’ll review your requested appointment, contact you to confirm scheduling and send paperwork within 2 business days, Monday-Thursday. We take all federal holidays off.
